- To submit a case for resolution please fill out the Issue Report Form. All issues must be reported within 10 days of their occurrence and resolved within 30 days.
Our team will be notified as you submit these forms, and a dedicated Sweeps Support Team member will be assigned to your Claim. They will review information submitted by you, and gather more information as necessary to make a decision.
We may be in touch with you again as part of this process and will communicate a decision with all parties in a timely manner. Final agreements to resolutions must be submitted at the Sweeps Resolution Form.
